Sutter Health Community Health Needs Assessment Focus Group

Sutter Roseville Medical Center and Sutter Auburn Faith Hospital have contracted with Community Health insights to conduct a 2025 community health needs assessment (CHNA). The objective is to identify and prioritize community health needs—defined as the basic provisions and conditions needed for the improvement and/or maintenance of health—for the communities served by the hospitals.
The final deliverable of this project will be a written report detailing the CHNA that will be posted on the hospital’s website. Based on the results, the hospitals develop community health improvement plans or implementation strategies to address the health needs that were identified.
As a valued member of the Placer Collaborative Network, you have an opportunity to participate in the CHNA process by sharing your insights and perspectives during our upcoming meeting. Focus group participants will be asked to share their thoughts about the health needs and challenges in the communities they serve in Placer County. There is NO PREP needed prior to the focus group.
At the conclusion of the focus group, there will be a drawing held for 3 gift cards for those that participated in the focus group.
You may have already participated in the 2025 CHNA process by participating in a one-on-one interview. If so, thank you for being part of this important work and feel free to skip this meeting, or send a staff member in your place to this share their insights.
